Newspaper notice — publish this first in a daily local leading newspaper:

The Word file has the proforma without the witness part and your old name typed in place of the signature, as the guidelines ask. Copy it to a CD.

Print, sign in your old name, and get two witnesses to sign both copies of the proforma.

What to send, in order

  1. Newspaper: publish the notice (text above) in a daily local leading newspaper — it must give your father’s / husband’s name and address. Keep the original newspaper.
  2. Fee: pay ₹1,100 on bharatkosh.gov.in (₹1,350 if you take a name of another religion without changing religion). Note the transaction number.
  3. Print and sign this PDF: request letter, undertaking, declaration on genuineness, the proforma twice (each with two witnesses), and the certificate. Sign in your old name.
  4. CD: copy the Word file onto a CD.
  5. Add two self-attested passport size photos and a self-attested copy of your photo ID.
  6. Send or hand in everything to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054 — in person or by post. Agents and advocates are not accepted, and documents are not returned. Documents should not be older than one year.
  7. After publication: download the notice from egazette.gov.in → weekly Gazette, Part IV. No printed copy is sent.

What documents are needed for a gazette name change?
For an adult in India: an undertaking, the original newspaper with your change-of-name notice, the proforma in duplicate signed by you and two witnesses, a CD with the proforma in MS Word, two self-attested passport photos and a self-attested photo ID, a certificate that the hard and soft copies match, and a request letter with the fee. What is the fee for a name change in the Gazette of India?
₹1,100 for an applicant in India, paid through Bharatkosh (NTRP). It is ₹1,350 if you take a name of another religion without changing your religion. Indians living abroad follow a separate guideline.
Where do I send the gazette name change documents?

Do I sign in the old name or the new name?
In your old name — the proforma says “Signature (in existing old name)”. In the Word file for the CD, your old name is typed where the signature goes.
How do I get a copy of the gazette notice?
The Gazette is published only online. Go to egazette.gov.in, open the weekly Gazette, Part IV, for the week your notice appears, and search for your old or new name in the PDF.
Is my data uploaded?
No. The papers and the Word file are made in your browser; nothing is sent anywhere.
