Which Voter Form Do I Need?

SIR notice: the 12 documents the ECI accepts

Needed only if the ERO sends you a notice — not when you fill the enumeration form. Any one that applies:

  1. Identity card or pension payment order of a central / state government or PSU employee or pensioner
  2. Any identity card, certificate or document issued in India before 1 July 1987 by the government, local bodies, banks, post office, LIC or PSUs
  3. Birth certificate from the competent authority
  4. Passport
  5. Matriculation or other educational certificate from a recognised board or university
  6. Permanent residence certificate from the competent state authority
  7. Forest Rights Certificate
  8. OBC / SC / ST or other caste certificate from the competent authority
  9. National Register of Citizens (where it exists)
  10. Family register prepared by the state or local authorities
  11. Government land or house allotment certificate
  12. Aadhaar (as per ECI letter No. 23/2025-ERS/Vol.II dated 09.09.2025)

Where to apply

  • Online: voters.eci.gov.in (log in with your mobile number) or the ECINet app.
  • Offline: your Booth Level Officer (BLO) or the ERO office. Blank forms are with the BLO.
  • Helpline: 1950. Keep every acknowledgement and notice — you need them if you appeal.

Questions people ask

Which form do I fill to add my name to the voter list?
Form 6 — for a first-time voter, or if your name is missing. You can apply at 17 in advance; the name is added when you turn 18 on 1 January, 1 April, 1 July or 1 October.
Which form is for change of address in the voter ID?
Form 8. Since August 2022 the same Form 8 is used for shifting house, correcting details, a replacement card and marking a person with disability. Apply to the ERO of your new address.
What documents do I need in the SIR?
None when you fill the enumeration form. Documents are needed only if the ERO sends you a notice — then one of the 12 documents the ECI accepts, such as a birth certificate, passport, school-leaving certificate or Aadhaar.
My name was removed. How do I appeal?
Appeal to the District Election Officer (District Magistrate) within 15 days of the ERO’s order, and if needed to the state Chief Electoral Officer within 30 days of that order, under section 24 of the Representation of the People Act, 1950.
What is Form 7 for?
To object to a name or ask for its deletion — for example a family member who has died, a duplicate entry or someone who has moved away permanently. The ERO checks it before deleting.
