Check what the office accepts
Offices such as UIDAI, the passport office, schools and banks each list the documents they accept as proof of date of birth, and some have their own affidavit format — use theirs if they give one. A birth that was never registered is registered through the local registrar of births, not by affidavit alone. State only what is true.
