When it is used

When your name is written differently on two documents — a middle name left out, a short form, a spelling difference — offices may ask for an affidavit that both names belong to you. Some offices also want the record itself corrected (for Aadhaar, PAN or school records, see the Name / DOB mismatch checker). If an office gives its own format, use that.

What is a one and the same person affidavit?

A sworn statement that two (or more) differently written names are of the same person — you — with the documents where each appears.

Does it need stamp paper and a notary?

Usually it is signed before a notary or oath commissioner on stamp paper; the value depends on the State. Ask the notary.

Can it change my name in records?

No — it explains the difference. To change a record, apply to the office that keeps it; for a full change of name there is the Gazette process.
